About the Role
As a Registered Nurse (RN) at Cardinal Woods Skilled Nursing and Rehab, you will play a vital role in delivering exceptional patient care to our short-term rehabilitation and therapy patients. You will work closely with our interdisciplinary team of physicians, nurse practitioners, nurses, and state-tested nursing assistants to develop and implement individualized care plans that cater to each patient's unique needs.
Responsibilities
Provide direct patient care, including assessments, medication administration, and wound care, in accordance with established standards and protocols
Develop and implement individualized care plans in collaboration with the healthcare team
Monitor patient progress, identify areas for improvement, and adjust care plans as needed
Communicate effectively with patients, families, and healthcare professionals to ensure that patients receive comprehensive care
Participate in quality improvement initiatives and contribute to the development of evidence-based practice guidelines
Maintain accurate and detailed records of patient care and treatment
