About the Role

As a Registered Nurse (RN) at Cardinal Woods Skilled Nursing and Rehab, you will play a vital role in delivering exceptional patient care to our short-term rehabilitation and therapy patients. You will work closely with our interdisciplinary team of physicians, nurse practitioners, nurses, and state-tested nursing assistants to develop and implement individualized care plans that cater to each patient's unique needs.

Responsibilities

• Provide direct patient care, including assessments, medication administration, and wound care, in accordance with established standards and protocols

• Develop and implement individualized care plans in collaboration with the healthcare team

• Monitor patient progress, identify areas for improvement, and adjust care plans as needed

• Communicate effectively with patients, families, and healthcare professionals to ensure that patients receive comprehensive care

• Participate in quality improvement initiatives and contribute to the development of evidence-based practice guidelines

• Maintain accurate and detailed records of patient care and treatment
