Licensed Clinical Social Worker (LCSW) - 26-65

Job Description Licensed Clinical Social Worker – Locations: San Ramon, San Joaquin, Sacramento, California – Full time. Responsibilities Provide telephonic psychosocial assessments and interventions to assist patients and caregivers during care transitions and while coping with illness, diagnosis, treatment, and life situations. Educate members, families, and caregivers regarding mandatory self‑management related to diagnosis, illness, and treatment. Address biopsychosocial stressors related to disease management to improve members’ overall well‑being and reduce ED utilization and readmissions. Address barriers to social determinants of health (SDOH) and connect members to community programs. Coordinate and collaborate with hospitals, skilled nursing facilities, home health, hospice, and other community agencies to advocate for members’ ongoing care needs. Participate in inter‑disciplinary team meetings to discuss members’ needs and address care gaps. Assist in advance care planning and completion of Advance Directives and POLST. Coordinate palliative care services and/or end‑of‑life services where appropriate. Required Experience LCSW license. 2–4 years of social work experience within the last five years (mandatory fieldwork not included). Experience in a managed care environment preferred; hospital experience preferred. Salary Range $88,700 – $123,350 annual.
