Job Description:
Client Support, Service Coordination, and Navigation
- Assist clients with completing forms, applications, eligibility documents, and referrals for healthcare and community services.
- Help individuals and families navigate medical, WIC, social determinants of health (SDOH), mental/behavioral health, and substance use disorder (SUD) services.
- Participate in care coordination and case management activities as appropriate.
- Maintain regular communication with clients through telephone, text messaging, or in-person visits.
- Conduct follow-ups to support closed-loop referrals and ensure clients are connected to needed services.
- Maintain current referral pathways for internal and external healthcare and community resources.
- Provide culturally sensitive and client-centered support to diverse populations.
- Assist with administrative activities related to client navigation and outreach events.
Maternal and Child Health Support
- Provide education and support related to maternal, infant, and child health.
- Deliver education on Safe Sleep, Car Seat Safety, perinatal health, and postpartum health.
- Conduct standardized 30-, 60-, and 90-day follow-ups with clients and families.
- Support families during pregnancy, postpartum, and early childhood periods.
- Provide education and resources related to infant wellbeing and substance exposure.
- Support referrals and coordination with substance use disorder programs and other community partners.
- Assist families with Plans of Safe Care as appropriate.
- Provide education, resources, and referrals related to pregnancy loss.
- Maintain pregnancy loss educational materials and resource information.
- Assist with tracking and reporting activities related to pregnancy loss programming.
- Strengthen referral pathways among WIC, MCH, SUD, healthcare, and community service partners.
Community Health Education
- Provide individual and community-based health education focused on disease prevention, health promotion, maternal and child health, and healthy behaviors.
- Educate individuals and families on how to access and navigate healthcare and social service systems.
- Develop, obtain, and distribute educational materials from credible healthcare and community sources.
- Provide education using plain language and culturally appropriate communication methods.
- Support health literacy and cross-cultural communication.
- Participate in community coalitions, groups, listening sessions, and health-related activities.
- Respond to requests from community organizations for information regarding health department programs and services.
- Support community initiatives addressing health disparities and SDOH.
Community Outreach and Engagement
- Build and maintain positive relationships with community members, organizations, healthcare providers, schools, coalitions, and other partners.
- Attend community outreach events, partner meetings, and listening sessions.
- Collect community surveys, feedback, and information regarding community needs and strengths.
- Participate in strengths-based and appreciative inquiry activities.
- Serve as a liaison between health district programs, community organizations, schools, healthcare providers, and other local agencies.
- Utilize evidence-based strategies to support community health improvement.
- Support initiatives designed to improve health equity and reduce disparities.
- Assist with quarterly reporting of community feedback and engagement activities.
Infant Wellbeing and Substance Exposure Support
- Provide education regarding infant wellbeing and substance exposure.
- Connect families with appropriate healthcare, behavioral health, and substance use disorder resources.
- Support referrals and partnerships with SUD programs.
- Assist with Plans of Safe Care and related family support activities.
- Provide follow-up and resource navigation to promote continuity of services.
Program and Administrative Support
- Document client outreach, education, referrals, follow-ups, and community engagement activities.
- Maintain accurate program records and documentation.
- Support tracking of participation, referrals, client outcomes, and program activities.
- Maintain educational materials, supplies, and outreach resources.
- Assist with MCH program communication, reporting, timelines, and deliverables.
- Support coordination of CHW education programs and promote consistency in program curricula.
- Assist with program evaluation and assessment of CHW services and community impact.
- Participate in community health assessments and improvement planning activities as assigned.
- Assist with required local, state, or federal program reports.
- Support communication of program results and findings to appropriate stakeholders.
- Provide basic language interpretation and navigation support using approved VDH language access resources and materials.
- Participate in required training and professional development.
General Public Health Responsibilities
- Support disease prevention, health promotion, health equity, and public health initiatives.
- Collaborate with multidisciplinary teams and community partners to identify and address community health needs.
- Maintain confidentiality and follow applicable VDH policies, procedures, and requirements.
- Utilize technology and internet-based tools for documentation, communication, data entry, and research.
- Support outreach and educational supply inventory management.
- Perform other duties as assigned to support public health programs and services.
- During a public health emergency, may be required to adjust work hours, work location, and job duties based on VDH business needs.
Minimum Knowledge, Skills, and Abilities
- Knowledge of social determinants of health and factors affecting access to healthcare and community resources.
- Knowledge of basic mental and behavioral health concepts and their relationship to overall health.
- Knowledge of public health principles, health promotion, disease prevention, and community health practices.
- Understanding of healthcare, social service, and community resource systems.
- Experience working with underserved, priority, or diverse populations.
- Demonstrated cultural competency and ability to work respectfully with individuals from diverse backgrounds.
- Strong verbal and written communication skills.
- Ability to communicate clearly using plain language.
- Strong interpersonal and relationship-building skills.

- Bilingual proficiency in Spanish or another language commonly spoken in the community.
- Previous Community Health Worker experience.
- Community Health Worker certification or willingness to obtain certification.
- Experience working with maternal and child health populations.
- Knowledge of maternal, perinatal, postpartum, and infant health.
- Experience working with underserved or priority populations.
- Experience in public health or a community-based agency.
- Knowledge of social determinants of health and community resource navigation.

Work Environment and Physical Requirements
- Work will be performed primarily at the Staunton Office, with travel to Waynesboro, Harrisonburg, Lexington, and community locations as needed.
- Driving is an essential job function.
- The position may involve both office-based and community-based activities.
- The contractor may be required to attend community events, meetings, outreach activities, and home or community visits as appropriate.
- VDH will provide a laptop and cellphone for work-related duties.
Special Assignments and Emergency Responsibilities
The contractor may be required to perform other duties as assigned. In the event of a public health emergency or emergency declaration, VDH may require adjustments to work hours, work location, responsibilities, or training to meet public health business needs. The contractor may periodically be required to report to the assigned office for meetings, training, or other operational needs.
