Work Environment

This position is an onsite role within a home care environment, where you will provide therapy to patients within their own homes. Key Responsibilities

• Direct and implement physical therapy treatment plans aimed at preventing disability, alleviating pain, restoring muscle function, and maximizing the client's performance in everyday activities based on physician prescriptions.
• Supervise and mentor Physical Therapy Assistants to ensure fidelity in the implementation of care plans and adherence to best practices in therapy treatments.
• Conduct comprehensive evaluations using diagnostic tests to assess clients' physical capabilities and develop tailored rehabilitation strategies.
• Assess clients' home environments to recommend modifications that boost their independence and safety during rehabilitation.
• Utilize a range of therapeutic exercises and modalities such as traction and massage to provide treatment, alongside educating and motivating clients in their rehabilitation programs.
• Fit, evaluate, and recommend adjustments for prosthetic and orthotic devices to optimize client mobility.
• Participate in care plan development and regularly update treatment protocols in collaboration with the interdisciplinary team.
• Document clinical evaluations, create thorough care plans, and maintain progress notes to ensure comprehensive client records.
• Facilitate effective communication with clients, family members, and healthcare personnel to enhance the holistic management of client care.
• Ensure timely submission of documentation and adhere to branch policies on clinical and payroll records.
• Complete periodic functional assessments and contribute to the utilization and coordination of community resources for client support.

Patient Population / Client Focus

You will directly impact patients of diverse ages and backgrounds, ranging from individuals recovering from surgery to those with chronic illnesses requiring long-term physical rehabilitation. The emphasis on personalized, compassionate care ensures that every client receives the attention and resources necessary for effective recovery.

Qualifications

• A Doctorate in Physical Therapy (DPT) is essential for this role.
• Must hold an active and unblemished Physical Therapist license in Florida.
• Demonstrated expertise in various manual therapy techniques and exercise program development.
• Emphasis on research-supported physical therapy practices and the ability to utilize electronic medical record (EMR) systems such as Web PT effectively for patient documentation.

Preferred Experience

• Previous home health care experience is highly desirable and can enhance your effectiveness in this role.
• Experience managing clients with diverse health conditions in both residential and outpatient settings.

Compensation and Benefits
