Contract | Days | Field-Based Position (Tuesday–Saturday, Wednesday–Sunday, or Monday–Friday) | 5x8-Hour Shifts

Key Responsibilities

• Provide physical therapy services to patients in a home health setting across an assigned territory
• Complete patient visits with timely documentation and communication
• Perform and document OASIS assessments as required
• Participate in on-call rotation as needed
• Complete required orientation modules during onboarding

Required Qualifications

• Active California Physical Therapist license required at time of submission (pending CA license accepted if application has been submitted)
• BLS certification through the American Heart Association (AHA) required
• Active driver's license and personal transportation required
• OASIS experience required
• Minimum of 1 year of Home Health experience
• Local candidates are welcome and will be considered at travel rates

Preferred Experience / Skills

• Prior home health experience strongly preferred; candidates without home health experience may be considered
• Familiarity with home health compliance documentation and checklists
