Description Licensed Practical Nurse (LPN) – Care Coordinator Location: Benton, Arkansas Employment Type: Part-Time Work Location: In-Person Schedule: Monday-Friday; Flexible Hours Job Summary Family Practice Associates is seeking a dependable, compassionate, and organized Licensed Practical Nurse (LPN) Care Coordinator to join our busy family practice team. The Care Coordinator plays an important role in helping patients transition safely between healthcare settings, supporting providers with ongoing patient care, and improving health outcomes through quality initiatives. This position will work closely with providers, clinical staff, patients, and outside healthcare organizations to ensure appropriate follow-up and continuity of care. The ideal candidate is a self-motivated team player who is comfortable working independently, managing multiple priorities, communicating effectively with patients, and following through on tasks in a fast-paced medical practice. Essential Duties and Responsibilities Contact patients following hospital admissions, inpatient stays, emergency room visits, and other significant healthcare encounters to facilitate timely follow-up. Perform and document medication reconciliation , identifying discrepancies and communicating concerns to the provider as appropriate. Contact patients to assess and encourage medication adherence and assist with identifying barriers to treatment. Coordinate and schedule appropriate follow-up appointments, diagnostic testing, referrals, and other recommended care . Participate as an integral member of the practice's Quality Team . Identify and assist in closing patient care gaps and preventive health needs. Monitor patient follow-up and assist in ensuring recommended care is completed. Make patient outreach calls and document all appropriate communication and interventions in the electronic medical record. Assist with population health and quality-improvement initiatives, including tracking and following up on patients who may benefit from additional services. Communicate effectively with patients, caregivers, providers, pharmacies, specialists, hospitals, and other healthcare organizations as appropriate. Maintain patient confidentiality and comply with HIPAA and all applicable laws, regulations, and practice policies . Provide compassionate, professional, and respectful patient interactions. Perform other duties within the scope of the LPN license and as assigned by practice leadership. Requirements Qualifications Required Current, active, and unrestricted Licensed Practical Nurse (LPN) license in the State of Arkansas. Strong organizational and time-management skills. Ability to prioritize and manage multiple responsibilities in a fast-paced environment. Excellent verbal and written communication skills. Strong attention to detail and follow-through. Ability to work effectively both independently and as part of a team. Dependable, professional, and committed to providing excellent patient care. Ability to maintain patient confidentiality and handle sensitive information appropriately. Preferred Previous experience in a family practice, primary care, or ambulatory healthcare setting. Experience with care coordination, transitions of care, population health, or quality improvement initiatives. Familiarity with healthcare quality measures, preventive care guidelines, and care-gap closure. Experience working with an electronic health record (EHR). Physical Requirements Ability to sit and work at a computer for extended periods. Ability to communicate effectively by telephone and in person. Ability to occasionally move throughout the clinic and perform other duties associated with a clinical office environment. Ability to perform the essential functions of the position with or without reasonable accommodation. Work Environment In-person position in a busy family practice clinic. Frequent interaction with patients, providers, clinical staff, and outside healthcare organizations.
