Licensed Practical Nurse (LPN)

Description of the role: We are seeking a Licensed Practical Nurse (LPN) to join our team at Pleasant Hill Adult Day Health Center and Senior Services. In this role, you will provide essential nursing care to our residents/clients, ensuring their health and well-being while supporting their journey.

Schedule:

1. Monday - Friday: 9:00 AM - 5:00 PM
2. One weekend per month (Saturday and Sunday) 8:00 AM - 8:00 PM

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Perform triage assessments and monitor patient vital signs.
• Collaborate with healthcare team members to develop and implement patient care plans.
• Educate residents and families on disease processes.
• Maintain accurate patient records.
• Supervisory visits in clients' homes regarding Certified Nursing Assistants/Personal Care Technicians.

Requirements:

• Current LPN license in the state of practice.
• Proven experience in a skilled nursing setting preferred.
• Strong communication skills for effective interaction with patients and families.
• Ability to perform clinical procedures competently.
• Compassionate demeanor with a commitment to patient-centered care.

Benefits:

We offer competitive compensation ranging from $25.00 to $29.00 per week, paid bi-weekly.
