Gifted Care Adult Family Home is a licensed, 4-bed residential care home serving adults with high-acuity, non-ambulatory care needs, including residents living with dementia. We're looking for a compassionate, dependable caregiver to provide hands-on personal care in a small, home-like setting where every resident gets individualized attention.

Responsibilities

• Assist residents with daily living activities, including bathing, grooming, dressing, toileting, incontinence care, and feeding.
• Perform safe transfers and repositioning using mechanical lifts and other assistive equipment, following each resident's care plan.
• Administer or assist with medications as trained and delegated, and document accurately on the MAR.
• Provide dementia-informed care and redirection, and engage residents in meaningful activities.
• Prepare meals according to dietary plans, and keep the home clean, safe, and organized.
• Observe and report changes in residents' condition, behavior, or skin integrity to the RN/owner promptly.
• Complete daily care documentation and follow all Wisconsin DHS 88 regulations, infection control practices, and emergency and fire safety procedures.
