Completes established competencies for the position within designated introductory period. Other related duties as assigned. General Functions:  Assesses human responses and adaptation to actual or potential health problems of the patient, family and/or significant other  Institutes and initiates a plan for patient care based on the initial assessment; implements and continuously evaluates interventions; reassesses patient needs on an on-going basis.  Recognizes changing patient condition and utilizes unit resources, chain of command and Rapid Response Team (RRT) in a timely and appropriate manner  Documents all plans and care provided  Identifies need for education of patient, family and/or significant other and provides education through individualized instruction, the use of written materials and/or other available resources  Assigns nursing care to licensed staff, delegates nursing care to unlicensed staff and coordinates the flow of work  Calculates and administers medications  Coordinates and collaborates with multidisciplinary team members in the planning and management of patient care  Participates in discharge planning; provides instruction to patient/family  Maintains and promotes professional standards and development by participating in inservices, committees, staff meetings  Serves as a role model for compassionate care and service excellence and commits to Studer Principles and “must haves”  Facilitates patient movement within the department and through the facility, proactively managing patient admissions, discharges and transfers.  Manages patient hand-offs at shift change and between department and facilities; utilizes documentation tools for hand-offs as required  Provides every patient the defined standard of care (ie; core measures, chest pain protocol)  Utilizes resources efficiently and effectively  Maintains safe environment  Addresses patient population—specific needs (clinical)  Participates in Performance Improvement Activities MINIMUM EDUCATION: Graduate of accredited school of nursing. PREFERRED EDUCATION: Bachelor's Degree MINIMUM EXPERIENCE: None for Med/Surg and Behavioral Health 1 year in acute/chronic dialysis for Dialysis PREFERRED EXPERIENCE: 2 years in acute care for Med/Surg 2 years in psychiatric facility for Behavioral Health 2 years acute/chronic dialysis for Dialysis REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State License for Registered Nurse. For RN in Behavioral Health: Non-Violent Crisis Intervention must be obtained within 60 days of hire or transfer into department PREFERRED CERTIFICATIONS/LICENSURE: REQUIRED CCOURSES/COMPLETIONS (e.g., CPR): Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire.
