As a Physical Therapist, you will collaborate closely with patients, their families, and multidisciplinary teams to ensure holistic care approaches. You will manage a flexible schedule, predominantly from Monday to Friday, while providing therapeutic interventions in various home settings that may include private residences, assisted living facilities, and hospice care environments.Key ResponsibilitiesYour responsibilities include but are not limited to:Directly overseeing the execution of physical therapy treatment plans aimed at preventing disabilities, relieving pain, and restoring muscle function.Conducting thorough evaluations of patients' function through diagnostic and prognostic testing, ensuring precise adaptations of therapy plans.Engaging with patients in their homes and evaluating their living spaces to develop strategies that foster independence and safety.Implementing therapeutic exercises and functional activities, providing hands-on instruction, motivation, and support to maximize patient engagement and recovery.Utilizing various therapeutic modalities, such as traction and massage, while assessing and recommending necessary adjustments to prosthetic and orthotic devices.Maintaining meticulous clinical documentation, including initial evaluations, progress notes, and discharge summaries, to uphold the continuity of care.Participating in case conferences with interdisciplinary teams to develop comprehensive treatment strategies that align with each patient's health care goals.Training and supervising Physical Therapy Assistants when applicable, ensuring compliance with state regulations and best practices.Establishing strong communication channels with patients and their families, providing education and resources to support their therapy journey.Patient Population / Client FocusIn this role, you will primarily serve patients who require rehabilitative services in the comfort of their homes, ranging from individuals recovering from surgery to those managing chronic conditions or terminal illnesses. Our diverse patient population includes elderly clients seeking to maintain functional independence, individuals recovering from acute injuries, and those in hospice care requiring compassionate support. Your expertise will greatly impact their healing processes and overall quality of life.QualificationsTo qualify for this position, candidates must possess:A Doctorate in Physical Therapy (DPT) from an accredited institution.A valid Physical Therapist license in the state of New Jersey.Current CPR certification.A valid driver's license and automobile insurance.A professional demeanor that embodies our organizational mission of excellence in patient care.Preferred ExperienceWe seek candidates with a minimum of 2 years of clinical experience, particularly within home care or acute care settings. Experience in rehabilitating elderly patients, as well as familiarity with OASIS documentation and interdisciplinary collaboration, is greatly appreciated.Compensation and BenefitsThis position offers a competitive yearly salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications.
