Work Environment

This full-time position operates primarily in patients' homes, allowing for a unique and intimate understanding of their goals and recovery processes. You'll work a Monday to Friday schedule, enabling a consistent routine while also granting you the flexibility to adapt patient treatments according to individual home care needs. Key Responsibilities

• Conduct comprehensive evaluations by reviewing pertinent medical histories and performing detailed assessments to determine suitable treatment plans.
• Effectively communicate evaluation findings and treatment strategies to patients and their families, involving them actively in the goal-setting process to enhance engagement and outcomes.
• Implement treatment protocols tailored to individual patient presentations, utilizing evidence-based practices and the latest advancements in the field.
• Collaborate closely with other healthcare providers when necessary, monitoring patient conditions and adjusting care plans to ensure optimal outcomes.
• Complete discharge planning and follow-up activities, ensuring that your documentation clearly articulates the patient's progression through therapy with timely and accurate notes.
• Manage time effectively to meet productivity expectations, maintaining an organized schedule and the necessary documentation of care provided.
• Contribute to departmental goals through ongoing professional development and engagement in marketing initiatives that enhance our community presence.
• Actively participate in creating strategies that remove barriers to patient access to care and improve patient experience through flexibility and adaptability.

Patient Population / Client Focus

You will work with a diverse range of patients who require tailored therapeutic interventions to recover from injuries, manage chronic conditions, or improve their overall mobility and function. The home care setting allows you to build meaningful relationships with patients, focusing on their unique needs and circumstances to provide holistic, empathetic care.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid DPT license in the state of New Jersey.
• Current American Heart Association's BLS for Healthcare Providers CPR certification.
• Strong commitment to patient-centered care and effective communication.

Preferred Experience

• While a minimum of two years of prior work experience is preferred, we welcome applications from new graduates eager to begin their careers in home health care.
• A background in geriatric care or experience working with individuals recovering from orthopedic surgeries and rehabilitation may be advantageous.
