Right Form Rehabilitation & Wellness Center in Matawan, New Jersey, is seeking a dedicated and skilled Physical Therapist to join our growing team. Work Environment

This role is based onsite but primarily involves visiting patients in their homes across Matawan and the surrounding areas. Key Responsibilities

• Deliver safe, effective, and compassionate patient care in an at-home setting, focusing on individualized treatment plans for each patient.

• Implement personalized treatment programs based on protocols established by collaborating Physical Therapists, ensuring thorough adherence to each patient's unique therapeutic journey.

• Conduct comprehensive patient evaluations, including historical information reviews, specific assessments, and the provision of clear recommendations while documenting results meticulously.

• Facilitate effective discharge planning and robust follow-up care, ensuring timely and accurate documentation of all patient interactions, including evaluation notes, interim progress summaries, and discharge evaluations.

• Engage in continuous learning and compliance by completing all necessary training modules and adhering to local, state, and federal regulations.

• Evaluate, treat, and direct therapy interventions for patients requiring physical therapy, skillfully documenting these medical needs and the resulting treatment plans clearly.

• Maintain open lines of communication with patients, families, fellow team members, and external parties, ensuring a collaborative approach to care and support.

• Participate in team meetings and share insights, as directed by management, contributing to the ongoing improvement of patient care standards.

Patient Population / Client Focus

Our primary focus is on serving individuals with various orthopedic conditions, including post-operative recovery, sports injuries, and chronic musculoskeletal issues. Physical therapists at Right Form Rehabilitation & Wellness Center will engage directly with a diverse patient demographic, tailoring treatments to their unique circumstances and rehabilitation goals, significantly contributing to their recovery and quality of life.

Qualifications

• Graduate from an accredited physical therapy program with a Doctorate in Physical Therapy (DPT).

• Possession of a valid Physical Therapy license in New Jersey, as well as any other certifications required by state regulations.

• Current certification in BLS for Healthcare Providers from the American Heart Association.

Preferred Experience

A minimum of two years of experience in a physical therapy role, preferably in home healthcare or outpatient settings, is highly valued. Experience working with orthopedic populations and proficiency in tailoring rehabilitation programs to individual patient needs will significantly enhance your application.
