Work Environment

This role primarily involves onsite patient care in their homes, allowing for a unique and personal approach to treatment. As part of our home care team, you will work closely with patients in various living conditions, requiring adaptability and strong interpersonal skills. Monday through Friday schedules provide you with a consistent work-life balance while ensuring that our patients receive uninterrupted care.

Key Responsibilities

• Conduct thorough evaluations that include reviewing medical history, performing detailed assessments, and documenting results to guide treatment plans.
• Communicate evaluation findings effectively with patients and their families, encouraging their involvement in goal-setting for their rehabilitation journey.
• Implement tailored treatment protocols based on patient evaluations, individual progress, and the latest advancements in physical therapy practices.
• Collaborate with multidisciplinary healthcare teams to optimize patient care, document any changes in patient conditions, and adjust care plans as necessary.
• Participate in discharge planning and follow-ups, ensuring comprehensive documentation of patient progress and treatment outcomes.
• Manage your schedule efficiently to meet productivity targets and participate in community outreach events to promote our services.
• Contribute to the department's goals by complying with healthcare regulations and taking initiative in your professional development.
• Actively engage with patients and their families to enhance their care experience and ensure access to necessary resources.

Patient Population / Client Focus

The Physical Therapist will work with a diverse patient population with various physical rehabilitation needs, ranging from post-operative recovery to chronic pain management. Delivering care directly in the patients' homes allows for a more personal connection, fostering trust and engagement in their treatment processes.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid state license to practice physical therapy is necessary to engage in home care services.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Strong communication skills with a focus on problem-solving and patient-centered care.

Preferred Experience

While a minimum of two years of experience in a similar role is preferred, incoming candidates with varied backgrounds are encouraged to apply. Compensation and Benefits

We are proud to offer a competitive salary range of USD 100,000 – 120,000 annually, commensurate with experience.
