Key Responsibilities

• Conduct comprehensive evaluations of patients, gathering detailed medical histories, performing thorough assessments, and formulating individualized treatment plans.
• Communicate evaluation findings and treatment plans clearly to patients and their families, ensuring that they are best equipped to participate actively in goal setting and recovery.
• Implement evidence-based treatment protocols tailored to each patient's unique needs, adjusting methods as necessary based on ongoing assessments and performance.
• Engage in collaborative discussions with other healthcare professionals regarding patients' conditions, identifying when modifications to the care plan are required.
• Oversee discharge planning and follow-up care, ensuring accurate documentation of treatment progress and outcomes that reflect the patient's course of treatment.
• Exhibit excellent time management skills by maintaining an organized schedule, promptly attending meetings, and achieving productivity benchmarks.

• Contribute to departmental goals, ensuring compliance with all regulatory requirements and accreditation standards.

Patient Population / Client Focus

Our primary focus is on providing high-quality physical therapy to patients across various demographics who are recovering from injuries, surgeries, or chronic conditions. Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid PT license in the state of New Jersey.
• Current American Heart Association's BLS for Healthcare Providers CPR certification is required.
• Prior work experience is preferred but not mandatory, making this an excellent opportunity for recent graduates or those new to the field.

Preferred Experience

• At least two years of experience in physical therapy, preferably in a home care or outpatient setting, is ideal.
• Experience working with diverse patient populations and varying rehabilitation needs is advantageous.
