Work Environment

As a full-time Physical Therapist in our home care setting, you will engage with patients in their homes, delivering tailored therapy services. This onsite position involves driving to various locations throughout the Red Bank area, allowing you to develop personal relationships with each patient and provide individualized care plans. You will work closely with a supportive administrative team and have access to state-of-the-art therapeutic techniques and equipment, enhancing the overall care experience.

Key Responsibilities

• Conduct thorough evaluations comprising patient assessments, pertinent history reviews, and goal-setting sessions with patients and their families to develop customized treatment plans.
• Implement evidence-based treatment protocols derived from evaluations, adjusting techniques based on patient responses and ongoing health assessments.
• Regularly consult with other healthcare professionals to ensure comprehensive patient care. Recognize and document any significant changes in a patient's condition that necessitate updates to their care plan.
• Prepare complete discharge plans and follow-up strategies, ensuring timely documentation of patient interactions and treatment progress.
• Exhibit effective time management skills by maintaining a structured schedule that aligns with productivity objectives while being punctual for meetings and patient care sessions.
• Engage in department initiatives that promote community-based marketing and outreach efforts to increase access to our services.
• Champion continuing education and professional development, mentoring students or colleagues and actively contributing to fostering a culture of learning within the team.
• Participate in initiatives aimed at removing access barriers for patients, collaborating with the team to understand and utilize patient satisfaction metrics.

Patient Population / Client Focus

Our primary patient demographic includes individuals recovering from surgery, managing chronic conditions, or needing rehabilitation after injuries. As an important part of our community-oriented care model, you will serve a diverse range of patients, ensuring that each receives compassionate, personalized, and effective therapy tailored to their unique needs.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid DPT license in the state of New Jersey.
• Certifications in Basic Life Support (BLS) for Healthcare Providers, as per American Heart Association guidelines.
• Ability to demonstrate excellent interpersonal skills and a passion for patient care.

Preferred Experience

• Minimum of two years of experience in physical therapy, particularly in home care settings.
• Experience working with multidisciplinary teams and a wide variety of patient conditions.
• Proficiency in using electronic health record (EHR) systems for documentation and patient management.
