Work Environment

This position is primarily conducted onsite within the home environment of our patients, ensuring that you can deliver care in a comfortable and familiar setting. The workweek consists of Monday to Friday, offering a structured schedule that allows for a balance between professional obligations and personal life. Key Responsibilities

As a Physical Therapist at Encompass Rehabilitation Center, your daily responsibilities will include:

• Directing individualized physical therapy treatment plans that address pain relief, disability prevention, and functional improvement.
• Overseeing and collaborating with Physical Therapy Assistants to ensure the successful implementation of treatment protocols.
• Conducting thorough evaluations using diagnostic tests to assess the client's physical abilities and limitations.
• Assessing clients' home environments and making necessary recommendations to enhance their independence and safety.
• Administering manual and non-manual therapeutic exercises in line with physician prescriptions.
• Providing guidance and education to clients and their families regarding therapy programs and the use of therapeutic devices.
• Participating actively in interdisciplinary team meetings to evaluate and update care plans collaboratively.
• Maintaining detailed clinical records, including evaluations, care plans, progress notes, and discharge summaries, adhering to compliance standards.
• Ensuring timely submission of clinical orders and documentation in accordance with organizational policies.

Patient Population / Client Focus

The focus of your work will be on diverse patients requiring rehabilitation services within their homes. You will interact with clients of various ages and conditions, from post-operative recovery to chronic illness management, always emphasizing patient-centered care that respects individual needs and preferences.

Qualifications

To be considered for this rewarding role, candidates must meet the following criteria:

• A Doctorate in Physical Therapy (DPT) is required.
• A valid state license as a Physical Therapist in New Jersey.
• Current CPR certification and a clean driver's license with reputable automobile insurance.
• Ability to work collaboratively within a team and demonstrate excellent interpersonal skills.

Preferred Experience

Compensation and Benefits
