Key Responsibilities

• Direct and implement individualized physical therapy treatment plans with the goal of preventing disability and alleviating pain, enhancing muscle function and maintaining the highest levels of performance for our clients.
• Oversee and mentor physical therapy assistants, ensuring the effective execution of prescribed care and compliance with treatment plans.
• Conduct comprehensive evaluations of client functionality through physical assessments, establishing baseline metrics to track progress effectively.
• Assess home environments for rehabilitation needs, recommending modifications that promote client independence and safety.
• Provide on-site therapeutic interventions, which may include administering manual therapy techniques, instructing clients in exercises, and applying physical modalities as indicated.
• Evaluate, fit, and provide guidance on prosthetic and orthotic devices, coordinating closely with physicians to optimize client support.
• Document clinical findings, progress notes, and care plans in compliance with industry standards and regulations, utilizing our electronic health records system.
• Collaborate with an interdisciplinary team during case conferences to discuss client treatment goals and strategies, ensuring a holistic and integrated approach to care.
• Educate clients and their families about the physical therapy process, building supportive relationships that foster patient engagement and adherence to care plans.

Patient Population / Client Focus

Qualifications

• Successful completion of an accredited physical therapy program.
