Position Overview

Silver Lane Healthcare Center is seeking a dedicated and skilled Physical Therapist to join our team on a full-time basis. In this role, you will play a pivotal part in the recovery process of our patients in a rehabilitation facility located in Woodbridge, New Jersey. You will work collaboratively with other healthcare professionals, utilizing your expertise to develop and implement rehabilitation plans tailored to individual patient needs.

Work Environment

You will work onsite in a state-of-the-art rehabilitation facility, where the atmosphere is supportive, dynamic, and focused on achieving the best clinical results. Our team comprises experienced healthcare professionals from various disciplines, providing a comprehensive approach to patient care. Your schedule will follow a consistent workweek, from Monday to Friday, with hours from 9 AM to 5 PM, allowing for a balanced work-life routine.

Key Responsibilities

• Assess patients through functional evaluations to identify and document movement limitations and establish baseline measurements.
• Develop and implement personalized therapy plans that align with each patient's specific rehabilitation goals, considering their strengths and areas for improvement.
• Manage a caseload of 1-2 patients per hour, focusing on delivering hands-on therapy sessions that may include manual therapy, exercise prescriptions, and patient education.
• Collaborate with occupational therapists, physicians, and nurses to ensure a seamless transition of care and optimal patient outcomes.
• Educate patients and their families about therapeutic interventions and techniques to support recovery at home.
• Regularly review and adjust treatment plans based on patient progress and feedback, ensuring adaptability in clinical approaches.

Patient Population / Client Focus

Your primary focus will be on orthopedic patients who require rehabilitation following surgeries, injuries, or chronic conditions. You will encounter a diverse patient demographic, including post-operative individuals, athletes recovering from sports-related injuries, and aged populations seeking to maintain mobility and independence. Each patient presents unique challenges and opportunities, allowing for creative and therapeutic interventions tailored to their specific conditions.

Qualifications

• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited educational institution.
• Current and active New Jersey Physical Therapy license (or eligibility) required to practice.
• Possess strong clinical, assessment, and diagnostic skills, coupled with the ability to communicate effectively in a team environment.

Preferred Experience

• A minimum of 2 years of experience in a physical therapy role, preferably within a rehabilitation or outpatient setting.
• Proficiency in utilizing electronic medical record (EMR) systems for documentation and patient tracking.
• Experience with a variety of treatment methodologies and techniques that promote patient recovery and rehabilitation.

Compensation and Benefits
