Work Environment

The Physical Therapist will work closely with a multidisciplinary team within our state-of-the-art rehabilitation facility in Carteret. Key Responsibilities

• Conduct comprehensive initial assessments to evaluate patients' physical capabilities and develop individualized treatment plans aimed at preventing disability and relieving pain.
• Implement and adapt physical therapy interventions, including therapeutic exercises, manual therapy techniques, and the use of physical agents, to optimize treatment efficacy.
• Supervise and mentor Physical Therapy Assistants, ensuring adherence to the devised plan of care while promoting best practices in service delivery.
• Engage in ongoing assessment of patients' progress and make necessary adjustments to treatment plans in consultation with physicians and the interdisciplinary care team.
• Document all clinical evaluations, treatment plans, and progress notes accurately and in a timely manner, following organizational standards and compliance requirements.
• Coordinate with patients and their families, providing education on the physical therapy process and home exercise programs to encourage active participation in their recovery.
• Assess and recommend modifications within the home environment to enhance the effective implementation of rehabilitation techniques and support patient independence.
• Participate in case conferences and interdisciplinary team discussions to align on patient care strategies and address any emerging challenges or adjustments in treatment plans.
• Engage regularly with the clinical team to review and develop care planning protocols, ensuring all documentation meets regulatory standards.

Patient Population / Client Focus

As a Physical Therapist at Flagship Healthcare Center, you will work with a diverse patient population, including individuals recovering from orthopedic surgeries, neurological conditions, sports injuries, and chronic pain issues. Qualifications

• A Bachelor's Degree in Physical Therapy or equivalent educational background.
• An active Physical Therapist license in New Jersey with no history of sanctions or board proceedings.

• Experience using electronic medical record (EMR) software, specifically WebPT, for precise documentation of patient care activities.
• Strong understanding of evidence-based physical therapy practices and a commitment to continuing education in the field.

Preferred Experience

• Prior experience in a rehabilitation facility or outpatient therapy setting is highly desirable.
• Experience working collaboratively within an interdisciplinary team and engaging with various healthcare professionals.

Compensation and Benefits
