Key Responsibilities

• Conduct thorough evaluations of patients, which include detailed reviews of their medical history, assessments, and the formulation of comprehensive treatment plans based on evaluation findings.
• Effectively communicate evaluation outcomes and treatment strategies to both patients and their families, actively engaging them in the goal-setting process.
• Implement personalized treatment protocols tailored to the specific needs of each patient, adapting as necessary in response to their progress and changes in the healthcare landscape.
• Collaborate with a multidisciplinary team of healthcare professionals, recognizing and documenting any changes in patients' conditions that necessitate updates to their care plans.
• Ensure timely discharge planning and follow-up care, accurately documenting all patient interactions and treatment progress to maintain a clear medical record throughout the patient's journey.
• Exhibit excellent time management skills by adhering to productivity expectations and maintaining punctuality for all professional commitments.
• Engage in hospital and community marketing initiatives to promote our services and patient successes.
• Actively participate in achieving departmental goals and compliance with relevant regulations, enhancing the overall quality of care.

Patient Population / Client Focus

Our clinic serves a diverse patient population, providing vital rehabilitation services to individuals recovering from surgeries, injuries, and chronic health conditions. Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid state license to practice physical therapy in New Jersey.
• Certification in Basic Life Support (BLS) from the American Heart Association is mandatory.
• New graduates are welcome to apply; however, a minimum of two years of related experience is preferred.

Preferred Experience

• Experience in outpatient rehabilitation settings is highly desirable.
• Familiarity with electronic health record (EHR) systems and rehabilitation documentation practices.
• Prior involvement in interdisciplinary healthcare teams and patient education initiatives is a plus.
