Key Responsibilities

• Design and implement individualized physical therapy treatment plans aimed at preventing disabilities, alleviating pain, and enhancing functional abilities in alignment with each patient's needs and goals.
• Oversight of Physical Therapy Assistants during the delivery of care, ensuring compliance with the established treatment protocols and providing mentorship as required.
• Conduct thorough evaluations of patients' functional capabilities using diagnostic assessments focusing on muscle, nerve, and joint function.
• Assess the home environment and recommend adaptations aimed at promoting independence and enhancing the quality of life for our clients.
• Deliver face-to-face physical therapy services to patients at the center and in their homes, ensuring treatments are administered in accordance with physician prescriptions.
• Utilize manual therapy techniques, motivational interviewing, and therapeutic exercises as part of the treatment process, adjusting approaches based on progress and patient feedback.
• Monitor patient responses to therapy and communicate necessary modifications to care plans to both patients and their healthcare team.
• Complete clinical documentation such as evaluations, progress notes, and discharge summaries, maintaining compliance with regulatory standards and organizational policies.
• Engage actively in interdisciplinary team meetings to formulate and review comprehensive care plans that encompass holistic treatment approaches.
• Educate patients and their families on therapy programs, proper use of equipment, and home exercise plans to promote continuity of care.
• Stay accessible for consultations with staff and families, ensuring open lines of communication during patient treatment periods.
• Participate in quality improvement initiatives and remain flexible to complete additional assignments supporting the organization's mission.

Qualifications

• A Bachelor's Degree in Physical Therapy or a related field from an accredited institution.
• Possession of an active Physical Therapist license valid in the state of New Jersey.

• Proficient in using electronic medical record (EMR) software for accurate documentation, preferably with experience in Web PT.

Preferred Experience

• Prior experience in a rehabilitation or outpatient setting, preferably with a focus on diverse patient cases.
• Experience leading or mentoring Physical Therapy Assistants.
