Key Responsibilities

• Conduct thorough evaluations of patients by reviewing medical histories, performing various assessments, and determining appropriate treatment plans along with documentation of results.
• Effectively communicate evaluation findings and treatment plans to patients and their families, involving them in the goal-setting process to enhance engagement and compliance.
• Implement treatment protocols with proficiency based on evaluation results and current patient needs, adjusting techniques as necessary according to the evolving healthcare landscape.
• Collaborate with other healthcare providers when necessary, documenting identifiable changes in a patient's condition that may impact care plans or require additional consultations.
• Facilitate timely discharge planning and follow-up care, ensuring accurate documentation of all patient interactions, including initial evaluations, daily encounter notes, interim progress summaries, and discharge evaluations.
• Demonstrate effective time management skills, meeting productivity expectations while maintaining an organized schedule and attending meetings punctually.
• Engage in community marketing activities to promote our services and foster relationships with local healthcare professionals and organizations.
• Actively participate in achieving department goals, adhering to compliance guidelines, and maintaining accreditation standards.
• Proactively pursue continuing education opportunities for professional growth, while serving as a clinical instructor for students as needed.
• Enhance patient experiences by being flexible and adaptable, taking initiative to remove barriers to care and engaging positively in access-related initiatives.

Qualifications

Compensation and Benefits
