You will be working within an onsite rehabilitation facility that promotes collaboration and teamwork among health care professionals. Key Responsibilities

• Assess and identify the objectives and requirements of each patient in a therapeutic setting, ensuring tailored treatment plans that address their individual needs.
• Manage an efficient caseload, attending to 1 patient per hour while adhering to the highest standards of care and professionalism.
• Leverage your prior experience in orthopedic outpatient environments to provide effective and evidence-based treatments that enhance patient mobility.
• Actively collaborate and consult with physicians, occupational therapists, and other healthcare professionals to optimize patient outcomes and care plans.
• Monitor and document the effects of therapeutic interventions and adjust treatment plans based on ongoing assessments and patient progress.
• Maintain comprehensive and accurate documentation including patient evaluations, progress notes, and billing information, in compliance with regulatory standards.

Patient Population / Client Focus

As a Physical Therapist at Transformative Rehab, you will serve a diverse client base, primarily focusing on patients with orthopedic conditions, post-surgical rehabilitation, sports injuries, and chronic pain management. Your role will be pivotal in promoting recovery, enhancing functional capabilities, and providing education to clients about their conditions and rehabilitation techniques.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program is required.
• A valid license to practice as a Physical Therapist in New Jersey.
• A minimum of 2 years experience in providing direct physical therapy services is essential.
• A clean professional record, free of board actions or disciplinary measures.
• Demonstrated proficiency in manual therapy techniques.
• Strong ability to work both independently and collaboratively within a team-oriented environment.

Preferred Experience

Experience in an orthopedic outpatient setting is highly valued, as is familiarity with physical therapy modalities and assessment tools. Candidates with additional certifications in specialized therapy techniques will be favored.

Compensation and Benefits
