Work Environment

This role is primarily based in clients' homes, allowing for a highly personalized treatment setting. Being onsite provides the opportunity to assess the patient's environment, leading to more effective therapy outcomes. You will be part of a supportive team environment where collaboration with other healthcare professionals, such as occupational therapists and physicians, is essential to delivering comprehensive care. The work schedule is structured from Monday to Friday, 9 am to 5 pm, promoting a healthy work-life balance.

Key Responsibilities

• Conduct comprehensive assessments to identify patient objectives and needs, designing personalized treatment plans accordingly.
• Deliver and manage physical therapy sessions effectively, ensuring the efficient treatment of one patient per hour.
• Implement cost-effective therapeutic interventions aimed at enhancing clients' mobility, strength, and overall health.
• Maintain clear lines of communication with patients and their families, keeping them informed of progress and adjustments to treatment plans.
• Document patient encounters in a timely manner, including evaluations, progress notes, treatment plans, and billing information, adhering to industry standards and regulatory requirements.
• Engage in continuous learning and professional development, staying current with best practices and innovations in physical therapy.

Patient Population / Client Focus

Your role will primarily involve working with a diverse patient population requiring home care rehabilitation. This may include individuals recovering from orthopedic surgeries, injuries, or chronic pain management. Understanding each client's unique circumstances will be essential to developing effective treatment strategies that facilitate recovery and improve daily functioning.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited educational program.
• A valid license to practice as a Physical Therapist in New Jersey.
• At least 2 years of hands-on experience in providing physical therapy services, particularly in orthopedic outpatient settings.
• A clean professional record with no disciplinary actions or board measures against your license.
• Proven proficiency in manual therapy techniques and therapeutic modalities.
• Strong organizational skills with the ability to work independently while thriving in a collaborative team environment.

Preferred Experience

While a background in home care is advantageous, we value diverse experiences that emphasize innovative approaches to patient care. Experience using electronic health record systems for documentation and patient tracking will be beneficial in maintaining accurate patient data and improving the efficiency of care delivery.

Compensation and Benefits
