Work Environment

The chosen candidate will primarily work onsite, visiting patients in their homes throughout the Perth Amboy area. Key Responsibilities

• Conduct thorough initial assessments of patients to identify their orthopedic needs, medical history, and rehabilitation goals.
• Develop, implement, and modify individualized treatment plans tailored to each patient, using evidence-based practices to ensure effective rehabilitation outcomes.
• Utilize advanced therapeutic techniques and modalities such as therapeutic exercises, manual therapy, and technology-driven tools.
• Engage in ongoing patient evaluations, tracking progress and making appropriate adjustments to treatment plans as necessary.
• Provide patients and their families with education on rehabilitation processes, pain management strategies, and prevention techniques for future injuries.
• Collaborate with a multidisciplinary healthcare team which may include physicians, nurses, and other therapists to provide integrated care solutions.
• Maintain accurate and timely documentation of patient encounters in compliance with regulatory standards.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A valid state license to practice as a Physical Therapist in New Jersey is essential.

Compensation and Benefits

Forge Physical Therapy Services offers a competitive salary ranging from $110,000 to $130,000 annually, commensurate with experience and qualifications.
