Work Environment

This role is fully onsite, focusing on home care, where physical therapists work directly with patients in their homes, creating a comfortable and personalized therapy environment. You will collaborate closely with other healthcare professionals to deliver coordinated and evidence-based care, ensuring that each patient's treatment plan aligns with their specific medical needs and lifestyle.

Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians, assessing their physical capabilities and therapeutic needs.
• Design, implement, and modify individualized treatment programs tailored to meet each patient's unique recovery goals.
• Utilize a variety of evidence-based therapeutic techniques, including manual therapy, exercise prescription, and patient education to optimize rehabilitation outcomes.
• Provide skilled in-home care that focuses on improving functional mobility while adhering to the care plans developed in conjunction with referring physicians.
• Demonstrate excellence in customer service, fostering strong therapeutic relationships with patients and their families.
• Collaborate with a multidisciplinary team to address complex cases and enhance patient outcomes.
• Consult and educate patients and caregivers on their treatment plans, empowering them to actively participate in their recovery processes.
• Maintain accurate and timely documentation in accordance with state regulations and organizational policies.

Patient Population / Client Focus

You will have the opportunity to work with a diverse range of patients, primarily focusing on orthopedic conditions, but also including pediatric, neurological, and sports medicine cases. This role is essential in fostering a supportive recovery environment that addresses both the physical and emotional aspects of rehabilitation.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A current and valid Physical Therapy License in the state of New Jersey.
• A minimum of two years of experience as a licensed physical therapist, with a focus on home care being highly desirable.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Exceptional verbal and written communication skills, enabling effective interaction with patients, families, and team members.
• Proficiency in word processing and electronic health record systems for documentation purposes.

Preferred Experience

• Experience with evidence-based treatment methodologies and a strong desire to enhance patient rehabilitation outcomes.
• Background in building and maintaining relationships with patients and the local community.
