Work Environment

This position is primarily onsite, focusing on home care services in Avenel, New Jersey, where you will interact directly with patients in the comfort of their homes. Key Responsibilities

• Conduct comprehensive assessments of patients to evaluate their functional limitations and design individualized treatment plans tailored to their specific needs.

• Implement evidence-based treatment protocols and therapeutic modalities that address the unique challenges faced by orthopedic patients.

• Regularly collaborate with other healthcare professionals to ensure a cohesive approach to patient care and adjust treatment plans based on evolving patient conditions.

• Document patient evaluations, progress notes, and discharge plans accurately and promptly to ensure compliance with regulatory standards.

• Maintain knowledge of state practice acts and adhere to the laws governing physical therapy practice and supervision.

• Participate in team meetings, contributing insights and engaging in initiatives aimed at improving access to care and patient outcomes.

• Ensure compliance with all corporate clinical training requirements, maintaining credentials and current practices.

Patient Population / Client Focus

Your primary focus will be on orthopedic patients, including individuals recovering from surgeries, injuries, or dealing with chronic pain conditions. You will employ your expertise to create customized therapy plans that involve exercise, manual therapies, and advanced rehabilitation techniques. This role allows you to build rapport with patients and support them on their journey to recovery within their own homes.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.

• Current state license to practice as a Physical Therapist in New Jersey.

• A minimum of two years of outpatient orthopedic experience.

• Current CPR certification to ensure a safe environment for clients.

Preferred Experience

• Experience working in a home care setting is highly desirable as it fosters a better understanding of the unique challenges faced by patients in their living environments.

• Familiarity with electronic medical records (EMR) and documentation software to streamline administrative responsibilities and improve patient tracking.
