Reliant Orthopedic and Sports is actively seeking a dedicated and passionate Physical Therapist (PT) to join our team in Sayreville, New Jersey. This full-time position focuses on delivering personalized physical therapy services within a home care setting. Work Environment

This role is primarily based onsite within the homes of our clients across Sayreville and the surrounding areas. Our physical therapists work closely with patients in their comfortable environment, fostering a therapeutic relationship that encourages healing and rehabilitation. You will collaborate with an interdisciplinary team of healthcare professionals, ensuring comprehensive care for each client.

Key Responsibilities

• Develop and implement tailored physical therapy interventions aimed at preventing disability, relieving pain, and restoring muscle function, in accordance with each client's individualized care plan.
• Supervise and guide physical therapy assistants in carrying out therapy plans, ensuring high standards of care.
• Conduct thorough evaluations of clients' physical capabilities through diagnostic assessments and observations in their home settings, determining effective rehabilitation strategies.
• Assess home environments to identify potential obstacles and suggest modifications to promote client independence and safety.
• Administer a variety of therapeutic techniques, including manual therapy and exercises, while educating and motivating clients about their rehabilitation exercises and overall wellness goals.
• Evaluate and adjust prosthetic and orthotic devices as needed, collaborating with healthcare providers for necessary adjustments.
• Maintain accurate clinical records, including initial evaluations, care plans, and progress notes, ensuring timely submission for client health documentation.
• Engage in interdisciplinary team meetings, sharing insights and collaborating with healthcare professionals to refine care strategies tailored to each client's needs.
• Consult with clients and their families, providing education on home care practices, rehabilitation strategies, and device utilization for ongoing support.
• Document all services rendered and adhere to clinical and payroll protocols, ensuring compliance with company policies.
• Conduct onsite assessments and develop active treatment plans in collaboration with other healthcare providers and community resources.
• Stay accessible to supervising Physical Therapy Assistants, ensuring high-quality care is maintained at all times.
• Assist with additional duties and projects aimed at fulfilling company objectives and enhancing the quality of service delivery.

Patient Population / Client Focus

The clients you will serve primarily include individuals recovering from surgeries, chronic pain sufferers, and those requiring rehabilitation due to injuries or disabling conditions. This diverse patient population necessitates a compassionate and adapted approach, with a focus on empowering clients to achieve their health goals in the comfort of their own homes.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• An active and impeccable state license as a Physical Therapist, free from any disciplinary actions.
• Strong expertise in various manual therapy techniques and exercise program development.
• Proficiency in electronic medical records (EMR) software, particularly WebPT, for accurate patient documentation.
• A deep understanding of current research and practices within the physical therapy domain.

Preferred Experience

• Experience in home health care settings is strongly preferred.
• Prior work with diverse patient populations and varied physical therapy needs will be considered an asset.
• Previous roles involving collaborative practice with interdisciplinary teams will enhance candidate suitability for this position.

Compensation and Benefits

We offer a competitive annual salary in the range of $100,000 to $120,000, commensurate with experience and expertise.
