Reliant Orthopedic and Sports is seeking a dedicated and compassionate full-time Physical Therapist (PT) to join our Rehabilitation Center in Carteret, New Jersey. This onsite position is situated within our state-of-the-art Rehabilitation Center located in Carteret, NJ. Key Responsibilities

• Direct and provide physical therapy treatment focusing on pain alleviation, functional recovery, and disability prevention, accurately adhering to individualized care plans.
• Oversee and mentor physical therapy assistants to ensure the effective execution of treatment plans, guaranteeing high standards of patient care.
• Conduct thorough evaluations utilizing diagnostic tests related to muscle, nerve, joint, and functional abilities to establish a comprehensive understanding of each client's condition.
• Assess the patient's home environment to identify potential barriers to rehabilitation and develop actionable recommendations to enhance their independence.
• Deliver in-home therapy services as specified by a physician's orders, ensuring the effective administration of therapeutic exercises and assistive methods.
• Implement a variety of manual therapy techniques along with appropriate use of traction and physical modalities to support recovery.
• Evaluate, fit, and make necessary adjustments to prosthetic and orthotic devices, working closely with the prescribing physician for any modifications.
• Participate actively in care plan development and updates, collaborating with an interdisciplinary team to evaluate overall treatment efficacy.
• Maintain accurate clinical records including evaluations, care plans, progress notes, and discharge summaries promptly, adhering to organizational protocols.
• Engage with clients and families, providing essential education on the use and care of therapy devices, and instructing on the overall therapy program to promote adherence.
• Complete functional assessments and deliver support necessary for maximizing community resource utilization as dictated by patient needs.
• Fulfill additional duties and responsibilities as assigned, aiming to meet the overarching goals and objectives of the organization.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program is mandatory.
• Must hold an active Physical Therapist license in the state of New Jersey, free of sanctions or disciplinary history.
• Exceptional proficiency in manual therapy techniques and an ability to develop successful exercise protocols tailored to individual patient needs.
• Familiarity with electronic medical record (EMR) systems, specifically Web PT, to ensure accurate and timely documentation.
• Deep knowledge of current physical therapy methodologies and practices, complemented by a commitment to continuing education.

Preferred Experience

• Experience working in a multidisciplinary team, contributing to collaborative treatment planning for patients.
• Demonstrated success in working with a variety of patient populations, including pediatric and geriatric clients.
• Prior involvement in home health settings or community-based rehabilitation programs is highly regarded.

Compensation and Benefits

We offer a competitive salary range of $100,000 to $120,000 annually, complemented by a comprehensive benefits package.
