Work Environment

You will work primarily onsite, engaging with patients in a dynamic home care facility setting. Key Responsibilities

• Conduct in-depth assessments of patients to understand their functional limitations, pain levels, and rehabilitation goals.
• Design and implement individualized rehabilitation plans based on the assessment findings, incorporating manual therapy techniques and therapeutic exercises tailored to each patient's needs.
• Regularly monitor and evaluate patient progress through effective tracking techniques and adjust treatment protocols as needed to ensure optimal recovery.
• Educate patients on injury prevention, recovery strategies, and exercises to assist in maintaining progress and enhancing their quality of life.
• Document patient progress and treatment plans accurately in compliance with regulatory standards using clinic-approved electronic health record systems.
• Collaborate with other healthcare professionals to coordinate treatment approaches that support the patient's overall wellness and rehabilitation goals.

Patient Population / Client Focus

In this role, your primary focus will be on adults aged 40 to 65 years old, who often present with a variety of orthopedic conditions, such as arthritis, post-surgical rehabilitation, sports injuries, and other musculoskeletal issues. You'll play a vital role in enhancing their functional mobility and improving their quality of life through personalized therapy interventions that address their unique challenges.

Qualifications

• Bachelor's Degree in Physical Therapy (DPT or MPT) from an accredited institution.
• Active Physical Therapist license in the state of New Jersey.
• A minimum of 2 years of clinical experience in outpatient orthopedic settings.

Preferred Experience

• Proficiency with WebPT and other electronic health record systems is a significant asset.
• Experience in manual therapy techniques and therapeutic exercise programs.
• A strong commitment to ongoing professional development and participation in Continuing Education Units (CEUs).
