Work Environment

This physical therapist position requires a hands-on approach, as care will be delivered onsite in patients' homes. Key Responsibilities

• Conduct thorough evaluations to determine patients' needs, including reviewing medical history, performing specialized assessments, and formulating comprehensive treatment plans.
• Engage patients and their families during the evaluation process, providing clear explanations of findings and collaboratively setting rehab goals.
• Implement treatment protocols tailored to the individual, utilizing evidence-based practices to ensure optimal outcomes as health conditions evolve.
• Collaborate with other healthcare professionals when necessary, documenting any changes in the patient's condition and adjusting care plans accordingly.
• Manage discharge planning and follow-up care effectively, ensuring that thorough documentation of each patient's treatment journey is completed promptly.
• Exhibit exceptional time management skills to meet productivity standards while maintaining an accurate and organized schedule conducive to home care operations.
• Contribute to department initiatives, complying with relevant regulations and ensuring that the highest standards of care are met.
• Engage in professional development opportunities, including serving as a clinical instructor and mentoring students in the field.
• Promote an excellent customer experience, adapting to the needs of patients effectively and participating in community outreach initiatives.

Patient Population / Client Focus

In this role, you will predominantly work with a diverse range of patients, including those recovering from surgery, managing chronic conditions, or regaining mobility after injuries. Your expertise will be essential in guiding individuals through their rehabilitation journey, focusing on building strength, improving mobility, and enhancing overall quality of life while delivering compassionate and personalized care in familiar home environments.

Qualifications

• Bachelor's Degree in Physical Therapy or equivalent.
• Current state licensure as required by New Jersey regulations.
• American Heart Association's Basic Life Support (BLS) for Healthcare Providers certification.

Preferred Experience

• A minimum of two years of experience in a physical therapy setting is strongly preferred, but motivated new graduates are also encouraged to apply.

Compensation and Benefits
