Key Responsibilities

As a Physical Therapist at Aligncare, you will:

• Develop and implement personalized treatment plans based on the patient's specific needs, limitations, and goals.
• Assess the extent of each client's functional abilities using diagnostic tools and monitoring methods.
• Supervise and guide physical therapy assistants in the execution of treatment plans to ensure effective patient care.
• Utilize therapeutic techniques including manual therapy, exercise prescription, and modalities, such as traction and massage, to optimize patient recovery.
• Continuously document patient progress through clinical records, including evaluations, notes, and discharge summaries, maintaining compliance with regulatory standards.
• Actively participate in interdisciplinary team meetings to discuss patient treatment plans and collaborate on approaches for best outcomes.
• Educate and empower patients and their families on the use and care of therapeutic devices, as well as their overall rehabilitation program.
• Regularly evaluate the home environment when necessary and suggest modifications for improving safety and independence for patients.
• Communicate effectively with physicians and case managers regarding patient progress and suggest adjustments to treatment as needed.
• Remain accessible to aid physical therapy assistants and other team members during patient treatments.

Compensation and Benefits
