This position is primarily onsite, as our therapists provide care directly in patients' homes throughout Lincroft, New Jersey, and surrounding areas. Key Responsibilities

• Conduct thorough evaluations of patients' physical health, including reviewing pertinent medical history, performing specific assessments, and determining appropriate recommendations and treatment plans.
• Articulate evaluation findings and treatment strategies to patients and their families, actively involving them in goal-setting processes to promote shared decision-making.
• Implement personalized treatment protocols based on patient evaluations, the dynamic nature of their conditions, and current best practices in healthcare.
• Collaborate effectively with multidisciplinary healthcare teams and other professionals to address patient needs, modifying care plans as clinical conditions change.
• Document all patient interactions and treatment activities meticulously in accordance with healthcare regulations, ensuring comprehensive tracking of patient progress and outcomes.
• Manage time effectively to meet productivity metrics while maintaining an organized schedule and timely attendance at meetings.
• Partake in marketing initiatives to enhance the visibility of our services within the community, contributing to our outreach efforts and brand presence.
• Engage in continuous professional development, taking the lead in pursuing learning opportunities and serving as a clinical educator for students in the field.
• Actively support initiatives that remove barriers to care access, employing analysis of site-level performance metrics to enhance service delivery.

Patient Population / Client Focus

As a Home Care Physical Therapist, you will work closely with a diverse patient population that includes individuals recovering from surgery, managing chronic conditions, or seeking rehabilitation following strokes or injuries. Your ability to tailor care and engage patients in their healing journey will be paramount in facilitating their recovery in a comfortable setting — their homes.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid state license to practice as a Physical Therapist in New Jersey.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is required.

Preferred Experience

• While a minimum of two years of clinical experience in physical therapy practice is preferred, we welcome new graduates who are eager and committed to learn within a supportive environment.

Compensation and Benefits
