Work Environment

This position is primarily onsite, where you will deliver care directly within clients' homes. Key Responsibilities

As a Physical Therapist at Reliant Orthopedic and Sports, your primary responsibilities will include:

• Directing physical therapy treatments tailored to each client's unique needs, focusing on preventing disability, alleviating pain, restoring muscle function, and enabling maximum performance.
• Supervising Physical Therapy Assistants, ensuring that care plans are executed effectively and appropriately.
• Conducting thorough evaluations of clients' functional abilities using diagnostic tests related to muscles, nerves, and joints.
• Assessing the home environment for rehabilitation potential, providing actionable recommendations to enhance clients' independence.
• Administering various therapeutic interventions, including manual exercises, traction, and massages, while instructing clients in non-manual exercises.
• Evaluating and modifying prosthetic and orthotic devices as necessary, in collaboration with physicians for optimal client care.
• Developing, reviewing, and updating personalized care plans collaboratively with interdisciplinary team members.
• Maintaining accurate clinical records, including evaluations, progress notes, and discharge summaries, to track client progress and inform future treatment.
• Engaging with clients and their families to provide education on therapy techniques, device usage, and overall rehabilitation goals.
• Participating as an integral member of the interdisciplinary team in regular case conferences to optimize treatment plans and enhance client outcomes.

Patient Population / Client Focus

You will work with a diverse client population within the home care setting, including individuals recovering from surgeries or injuries, those dealing with chronic pain, or patients requiring rehabilitation for various physical limitations. The personal and familiar setting of home care enables you to build rapport with clients, fostering trust and motivating them towards achieving their rehabilitation goals.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• An active Physical Therapist license in the state of New Jersey free of sanctions or board proceedings.
• Strong foundational knowledge and mastery in various manual therapy modalities.
• Exceptional skills in developing and implementing effective exercise programs tailored to individual client needs.
• Proficiency in using EMR software, particularly Web PT, for comprehensive documentation.
• A commitment to staying current with evidence-based physical therapy techniques and practices.

Preferred Experience

• Experience providing home care therapy services is highly desirable.
• Ability to work collaboratively with an interdisciplinary team and engage effectively with clients and their families.
• Demonstrated history of developing tailored therapeutic interventions that have led to improved patient outcomes.
