Key Responsibilities

• Conduct comprehensive evaluations of patients, including reviewing their medical history and performing specialized assessments to develop personalized treatment plans.
• Communicate evaluation findings and treatment plans effectively to patients and their families, involving them in goal-setting to promote engagement in their own recovery.
• Implement evidence-based treatment protocols aligned with each patient's specific needs and evolving healthcare practices, demonstrating adaptability as patient conditions change.
• Collaborate with a range of healthcare professionals to discuss patient progress, recognizing significant shifts in health that may require alterations to treatment strategies or additional consultations.
• Ensure timely and accurate documentation of patient care activities, maintaining thorough records that represent the course of treatment from initial evaluations through discharge planning.
• Manage time efficiently to meet productivity standards, keep appointments organized, and contribute to achieving departmental goals.
• Participate in the community and marketing activities aimed at enhancing service accessibility and raising awareness of our rehabilitation offerings.
• Engage in professional development through continuing education opportunities and mentorship, contributing to the overall competency of both self and the organization.
• Help remove barriers to care access, actively engaging in initiatives aimed at improving patient experience metrics.

Patient Population / Client Focus

As a Full-Time Physical Therapist at Upstream Life, you will work with a diverse patient population, including individuals recovering from surgery, sports injuries, chronic pain, and neurological conditions. You will be instrumental in assessing each patient's unique challenges and providing tailored therapeutic interventions that empower them to regain mobility, strength, and independence.

Qualifications

• A Bachelor's Degree in Physical Therapy or a Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current valid physical therapy license in New Jersey.
• American Heart Association's BLS for Healthcare Providers certification is required.
• Strong communication, organizational, and interpersonal skills.

Preferred Experience

• Minimum of 2 years of experience as a practicing Physical Therapist in an outpatient or rehabilitation setting is highly preferred.
• Experience in utilizing electronic health record (EHR) systems for documentation and patient management.
