Key Responsibilities

• Conduct comprehensive evaluations by reviewing patient histories and performing relevant assessments to formulate individualized treatment plans.
• Effectively communicate evaluation findings and proposed treatment plans to patients and their families, ensuring their involvement in the goal-setting process.
• Implement evidence-based treatment protocols tailored to patient needs, regularly adapting the approach as their conditions evolve.
• Collaborate with a multidisciplinary team of healthcare professionals to provide integrated patient care, recognizing the need for adjustments in the treatment plan based on documented changes in patient health.
• Document all patient interactions accurately and in a timely manner, including initial evaluations, daily progress notes, and discharge plans, to provide continuity of care and meet regulatory requirements.

• Engage in professional development activities, pursuing continuing education opportunities and serving as a mentor for less experienced staff and interns.

Patient Population / Client Focus

Qualifications

• Current certification in BLS for Healthcare Providers from the American Heart Association is required.

Compensation and Benefits
