Work Environment

This role is primarily onsite, providing therapy in the comfort of each patient's home. You will work a Monday to Friday schedule, amounting to 40 hours per week, allowing you to build meaningful relationships with patients while offering personalized care in their familiar environments. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, assessing their physical function and needs to develop tailored treatment plans.
• Implement evidence-based intervention strategies to facilitate recovery, including therapeutic exercises, manual therapy techniques, and modalities as prescribed in the patient's plan of care.
• Provide ongoing patient education, training, and consultation to empower individuals and their caregivers in managing their conditions effectively.
• Utilize patient tracking and management systems to document progress, adjust treatment protocols as necessary, and communicate effectively with the healthcare team.
• Demonstrate exceptional customer service and contribute to the overall effectiveness of interdisciplinary team meetings aimed at maximizing patient outcomes.
• Meet established minimum utilization goals while maintaining high standards of patient care and satisfaction.

Patient Population / Client Focus

In this role, you will primarily focus on orthopedic patients, offering specialized care to individuals recovering from injuries related to sports, surgery, or chronic conditions. You will also encounter diverse patient populations, including pediatric and neurologic cases, allowing you to broaden your experience across various specialties while delivering compassionate care tailored to each patient's unique needs.

Qualifications

• Graduate of an APTA or CAPTE-accredited physical therapy program.
• Possession of a valid State License in Physical Therapy.
• Basic Life Support Certification from the American Heart Association.
• Strong interpersonal skills, with a focus on building lasting relationships with patients and their families.
• Effective communication skills, both written and verbal, to ensure clarity and understanding in all patient interactions.
• Proficiency in word processing and electronic documentation systems.

Preferred Experience

While a Doctorate in Physical Therapy is preferred, we invite applications from those with a Bachelor's degree who have demonstrated a strong commitment to delivering quality patient care. Prior experience in home health, community-based therapy, or working with diverse patient groups will be considered advantageous.

Compensation and Benefits

Springleaf Healthcare offers a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience, along with a comprehensive benefits package.
