Work Environment

This role is onsite, focusing on home care, where therapists provide personalized therapy sessions in patients' residences. Key Responsibilities

• Conduct comprehensive evaluations of patients that include detailed medical history reviews and a variety of physical assessments to develop individualized treatment plans.
• Clearly communicate evaluation findings and treatment plans to patients and their families, actively involving them in goal setting to enhance patient engagement and motivation.
• Implement treatment protocols tailored to each patient's needs, drawing on evidence-based techniques, assessing patient progress, and modifying plans as necessary.
• Collaborate with fellow healthcare professionals for holistic care, documenting any changes in a patient's condition and suggesting adjustments in care when appropriate.
• Oversee discharge planning, ensuring proper follow-up care and timely documentation that reflects the patient's progress and ongoing needs throughout their treatment journey.
• Maintain an effective schedule that balances patient needs with productivity targets, demonstrating excellent time management skills.
• Engage in hospital and community outreach activities to promote physical therapy services and educate the public on the benefits of rehabilitation.
• Continuously develop professional skills, seeking opportunities for education and improvement, and act as a mentor for students or junior therapists.
• Contribute to departmental goals and objectives, ensuring compliance with all relevant regulations and accreditation standards.
• Facilitate a patient-centered approach that prioritizes excellent customer experiences, adaptability, and access to care.

Patient Population / Client Focus

Your primary focus will be on diverse populations, providing care to individuals recovering from surgeries, injuries, and chronic conditions. You will tailor your therapeutic approach to accommodate a wide range of physical capabilities and health conditions, ensuring that every patient receives the support they need to achieve their rehabilitation goals.

Qualifications

• Must possess a Doctorate in Physical Therapy (DPT) and hold a valid state license in New Jersey.
• Current American Heart Association BLS for Healthcare Providers CPR certification is required.
• A comprehensive understanding of current physical therapy practices and techniques.

Preferred Experience

• A minimum of two years of experience as a licensed Physical Therapist is required, demonstrating effective clinical skills in both outpatient settings and home care.
• Familiarity with electronic health record (EHR) systems and documentation processes that reflect best practices in patient care.

Compensation and Benefits
