Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical capabilities and challenges.
• Develop individualized treatment plans tailored to the specific needs and goals set forth by patients and their referring physicians.
• Implement and modify therapeutic exercises and interventions to optimize recovery, including but not limited to manual therapy techniques, functional training, and education on movement strategies.
• Maintain meticulous documentation of patient progress and treatment interventions, ensuring compliance with both company standards and regulatory requirements.
• Set and meet minimum utilization goals, contributing to the overall productivity of the home care team.
• Engage in ongoing professional development, participating in case reviews and team meetings.
• Educate patients and caregivers about treatment protocols, expected outcomes, and home exercises to encourage active participation in the healing process.
• Deliver exceptional customer service to ensure patient satisfaction and retention.

Patient Population / Client Focus

As a Physical Therapist at Oakbend, you will work with a diverse patient population that includes individuals recovering from orthopedic surgery, managing neurological conditions, or suffering from sports-related injuries. The role requires an understanding of various treatment modalities to effectively address the needs of children, adults, and the elderly, fostering an inclusive environment for all clients.

Qualifications

• A Doctorate (DPT) in Physical Therapy from an accredited program.
• A valid Physical Therapy license in the state of New Jersey.
• Basic Life Support (BLS) certification from the American Heart Association.

• Proficiency in utilizing electronic health records and other computer-based systems.

Preferred Experience
