Veterans Case Manager II: Housing & Recovery Advocate

U.S.VETS Phoenix seeks a Case Manager II to provide trauma-informed, strengths-based case management to veterans facing housing instability, trauma, and other barriers to stability. You will assess needs and develop individualized service plans addressing housing, employment, education, recovery, and health challenges. You will coordinate with the VA, landlords, employers, treatment providers, and community resources; conduct home visits; and support group work for diverse veteran populations, #J-18808-Ljbffr U.S.Vets
