Division/Program Summary The mission of the Bureau of Public Health Clinics is to promote a healthy community by providing New Yorkers with the resources needed to make informed and empowered health decisions; identify and treat tuberculosis and provide immunization and sexual health services regardless of ability to pay or immigration status. Position Summary Reporting to the Navigation Supervisor, the Public Health Advisor‑II/Patient Navigator will work in one of the Bureau's clinics and assist patients who are seeking HIV prevention services. Job Duties and Responsibilities Oversee caseload of STD clinic patients who are seeking HIV prevention services, ensuring that the patient is connected to prevention and treatment services and the patient adheres to the established plan of care. Deliver health education and health promotion messages according to STD clinical protocols. Coordinate logistics for plan adherence such as appointment reminders, scheduling, medication adherence, arranging transportation and other needed services. Collaborate with STD clinic staff to identify qualified candidates for the program, perform rapid HIV screening and provide results and advising in a confidential manner per bureau protocol. Maintain liaison with health care providers and others including public and private agencies to expedite the efficient follow up and treatment of patients enrolled in the program. Minimum Qualifications A baccalaureate degree from an accredited college, including or supplemented by twelve semester credits in health education, or in health, social or biological sciences. A baccalaureate degree from an accredited college, and six months of full‑time satisfactory experience in a health promotion or disease intervention/prevention program, performing one or more of the following: interviewing, conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data. A four‑year high school diploma or its educational equivalent, and four years of full‑time satisfactory experience as described in item 2 above. Education and/or experience equivalent to items 1–3 above. Undergraduate college credit can be substituted for experience on the basis of 30 semester credits from an accredited college for one year of full‑time experience. Additional Requirements: To be assigned to Assignment LevelII, candidates must have, in addition to meeting the minimum qualification requirements listed above, at least one year of experience as a Public Health Adviser, Assignment LevelI, or at least one additional year of experience as described in item2 above. 55a Program This position is also open to qualified persons with a disability who are eligible for the55‑aProgram.
