CAP Case Manager/ FT / Onslow & Duplin Counties

Job Title CAP Case Manager – Full Time, Onslow & Duplin Counties, North Carolina. Overview We are hiring for a CAP Case Manager who will provide critical case management services to beneficiaries at risk of institutionalization, ensuring their health and safety through person‑centered care planning and coordination of essential services. Responsibilities Assessment and Care Planning – conduct initial pre‑screening and assessments of beneficiaries and families to evaluate medical, psychological, behavioral, financial, social, cultural, environmental, legal, vocational, and educational areas; consult with beneficiaries and caregivers to educate about waiver services, Medicaid, and community resources; identify needs to prevent health and safety risks; develop individualized, person‑centered care plans; review and update plans at least every 12 months or when status changes; assist beneficiaries and families in understanding the plan of care. Coordination of Services – link beneficiaries and families to necessary services, equipment, and supplies; collaborate with community resources, healthcare providers, and agencies; initiate referrals and use community resources for planning and service coordination. Monitoring and Follow‑Up – provide ongoing monitoring via monthly phone calls and home visits, documenting observations and progress; evaluate effectiveness of care plans and recommend changes to achieve desired outcomes; maintain accurate, up‑to‑date case‑management documentation within the system, ensuring compliance with state and agency guidelines. Counseling and Support – provide emotional support and basic counseling to beneficiaries and families to strengthen support systems; assist families in navigating challenges including palliative care, behavioral issues, and medical needs. Documentation and Compliance – obtain documentation from medical staff; maintain medical records, ensuring documentation of current status, service changes, and referrals; ensure compliance with 10A NCAC 27G.0202 and other guidelines; complete required records per agency policy and State CAP manual, including discharge summaries; review billing codes for compliance. Collaboration and Advocacy – work with RNs and interdisciplinary team; participate in case discussions; advocate for program participants and families; serve as liaison between beneficiaries, families, and external providers; provide training and support to families to empower care management. Continuing Education and Professional Development – complete all state‑mandated training and agency‑required continuing education annually; stay current on CAP guidelines and best practices; update documentation systems as required. Other duties – all other duties as assigned. Supervisory Responsibilities This position has no supervisory responsibilities. Minimum Qualifications Strong understanding of Medicaid programs and compliance requirements. Excellent communication and interpersonal skills. Ability to manage multiple priorities and maintain detailed records. Experience working with medically fragile children or in pediatric healthcare settings and physically disabled adults with complex care needs. Education and Experience Social Work Background: Bachelor’s degree in social work; minimum one year of directly related community experience, preferably case management; completion of NC Medicaid‑certified training within 90 days. Human Services Background: Bachelor’s degree in human services or equivalent; minimum two years of community experience; completion of NC Medicaid‑certified training within 90 days. Non‑Human Services Background: Bachelor’s degree in a non‑human services field; two or more years of related community experience; completion of NC Medicaid‑certified training within 90 days. Nursing Background: Current North Carolina RN license with a two‑year or four‑year degree; at least one year of case‑management experience; completion of NC Medicaid‑certified training within 90 days. Physical Demands and Work Environment Lift minimum 10 lbs. Pull minimum 20 lbs. Squat, kneel, crawl, crouch, climb, stoop, stand, walk regularly. Benefits Payactiv – early access to earned wages.
