Job Overview We are hiring for: MHSU Adult & Child Care Manager . This hybrid, community-based position serves Wake/Johnson & surrounding counties. The role involves whole-person integrated care management for individuals with intellectual, developmental disabilities, and mental health and substance use challenges. Responsibilities Foster and facilitate the care management process. Conduct comprehensive assessments and facilitate tailored care management plans with the person supported and team members. Monitor ongoing care plan implementation and update the plan every 12 months or when significant change occurs. Implement workflows, policies, procedures, and documentation standards that promote whole-person care. Maintain a positive professional image in the workplace and community. Follow policies and procedures to ensure accountability of service delivery. Demonstrate passion for positive change by continuously improving and defining innovative care management interventions, staying up to date with best practices and informing the team of innovations. Collaborate cross-functionally with Integrated Care Team members, community organizations, and RHA departments to ensure successful care management implementation. Maintain a person-centered philosophy focused on strengths, needs, and preferences, fostering a culture that promotes health, wellness, and safety. Use data strategically to support effective, integrated care management services, ensuring data sharing and storage compliance with state and company policies. Implement all organizational health and safety policies and procedures to maintain a safe and healthy environment. Document services provided in a timely manner for proper reimbursement. Establish and maintain frequent and regular communication with persons supported, family members, team members, colleagues, and external partners to reinforce care planning goals and outcomes. Support an ethical culture in line with RHA Code of Conduct, investigating allegations per company policy and ensuring confidentiality and retaliation protection. Participate in Quality Improvement, Quality Assurance, and Human Rights committees, and perform other related duties as assigned. Qualifications Education: A master’s degree in a human service field plus at least one year of full-time experience with the MH/SU/IDD population; A bachelor’s degree in a human service field plus at least two years of full-time experience with the MH/SU/IDD population; A bachelor’s degree in a non‑human service field plus at least four years of full-time experience with the population. For care managers serving members with LTSS needs: must meet the above qualification and have at least two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience. Experience: care coordination, care management, or case management experience. Other requirements: valid driver’s license, preferred clinical license, reliable transportation. Following Policies and Procedures – understands and promotes adherence to company rules.
