Experienced Home Physical Therapist (PT)
Join University Home Care Inc as an Experienced Home Physical Therapist (PT) providing high-quality, personalized care to patients in the Southeast Region of MI. We are seeking a compassionate and skilled PT to help improve the health and well-being of our clients through tailored therapeutic interventions and support.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized treatment plans.
- Provide skilled physical therapy services in patients' homes to improve mobility, function, and quality of life.
- Document patient progress and report on outcomes to healthcare team and insurance providers.
- Educate patients and family members on exercises, equipment use, and safety precautions.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
Requirements
- Licensed Physical Therapist in the state of Michigan.
- Proven experience in home health physical therapy preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation to travel to patient homes in the Southeast MI Region and surrounding areas.
