Key Responsibilities\n

• Conduct thorough mental health assessments for patients and their families, taking into account their emotional state, life history, and specific concerns related to hospice care.

• Develop individualized treatment plans that prioritize mental and emotional health, aiding patients in coping with grief, loss, and anticipatory grief.

• Provide therapeutic interventions, including but not limited to grief counseling, bereavement support, and trauma-informed care approaches.

• Facilitate individual and family therapy sessions to navigate complex emotions and foster healthy communication among family members during challenging times.

• Respond to crisis situations with appropriate interventions, employing de-escalation techniques and making timely referrals to higher-level care when needed.

• Educate patients and their families about mental health issues, coping strategies, and the resources available to them locally and through our hospice facility.

• Maintain accurate and confidential documentation of assessments, treatment progress, and other essential records in compliance with HIPAA regulations.

• Participate in case conferences and interdisciplinary team meetings to share insights and promote a coordinated approach to patient care.

Patient Population / Client Focus\n

This position focuses on vulnerable patient populations, including individuals facing terminal illnesses, their families, and caregivers. You will support individuals navigating the emotional challenges of grief and loss while providing resources and clinically sound recommendations tailored to each unique situation.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current, valid LCSW licensure in the state of Nevada.

Compensation and Benefits\n

The compensation for this role ranges from USD 80,000 to 100,000 annually, depending on qualifications and experience.
