Job Summary The Transitions of Care Clinical Staff Pharmacist – Transplant is critical in ensuring patients are prepared to be transitioned to different levels of care and have the information necessary to take their medications safely, appropriately, and consistently. The pharmacist reviews medications prescribed for appropriate dosing, frequency and indications and addresses any discrepancies or recommendations with the patient’s primary team or prescriber. The pharmacist provides counseling on the medication regimen and resources to educate patients about instructions, uses, side effects and additional medication therapy management considerations. The pharmacist may also participate in specialty consults such as anticoagulation counseling, heart failure counseling, outpatient parenteral antimicrobial therapy and home infusion review, solid organ transplant and LVAD education and pill‑box fill, and discharge counseling for high‑risk patients discharging from UT Southwestern Clements University Hospital or Zale‑Lipshy Rehab facility. Required Experience and Education Education: Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy. General health and stress‑coping ability. Licenses and certifications: Upon hire, licensed pharmacist by Texas State Board of Pharmacy (TSBP); BLS basic life support certification with a course accredited by the American Heart Association (AHA). Preferred Completion of a PGY‑1 Pharmacy Practice Residency from an ASHP‑accredited residency program. Two years or more of clinical pharmacist experience. Job Duties Performs job responsibilities per organizational and departmental policies, procedures, performance improvement objectives, environmental/infection control standards, and state and federal laws and regulations. Performs all computer order entries accurately, thoroughly, and timely. Reviews and updates medication profiles and documents recommendations appropriately. Reviews medication orders, prescriptions, and home infusions for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety; seeks clarification if necessary and documents interventions or recommendations. Works independently and solves problems effectively with minimal supervision; makes decisions and uses good judgment; is cross‑trained and multifunctional, maintaining competency in multiple areas. Participates in cost‑effective use and selection of pharmaceuticals. Communicates drug information verbally and in writing to physicians, nurses, etc., courteously and professionally; assists in patient/family education and counseling regarding drug therapy and side effects; documents clinical activities demonstrating cost savings and concise communication for follow‑up. Assists in supporting pharmacy technicians with counseling and guidance. Identifies opportunities for improvement in the patient transitions of care process. Actively participates in educational programs, in‑services, meetings, orientations and training; takes on additional projects as needed. Demonstrates openness to variety within the work environment and consistently treats all customers respectfully; supports teamwork and cooperation with co‑workers; volunteers assistance to co‑workers. Participates in the clinical program; supports P&T initiatives, reviews patients on high‑risk medications, collects applicable data and communicates opportunities for system or team improvement; participates on committees or workgroups as assigned. Communicates effectively with patients regarding high‑risk medications and provides comprehensive medication management reviews for patients at high risk for re‑admission. Provides educational materials and in‑services for medical, nursing, and pharmacy team members. Assists with Joint Commission standards and pharmacy safety initiatives.
