Compensation

Competitive Pay

Annual earning potential of $100,000–$120,000+ , depending on caseload and productivity.

Mileage reimbursement available.

• Louisville
• Lafayette
• Erie
• Westminster
• Surrounding communities

What You'll Do

As a Home Health Physical Therapist, you'll:

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized treatment plans to improve strength, balance, mobility, and independence.
• Provide therapeutic exercises, gait training, balance training, and fall prevention education.
• Educate patients and caregivers on home exercise programs and safe mobility techniques.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
• Complete accurate electronic documentation.
• Monitor patient progress and modify treatment plans as needed.

Qualifications

• Active Colorado Physical Therapist License
• Graduate of an accredited Physical Therapy program
• Excellent communication and patient education skills
• Ability to work independently
• Reliable transportation and valid driver's license
• Home Health experience preferred, but we're happy to train clinicians who are eager to transition into Home Health.
