Responsibilities
- Provide physical therapy evaluations and develop treatment plans for residents in a skilled nursing setting
- Implement rehabilitation programs focused on mobility, strength, balance, gait, and pain management
- Document resident progress, adjust care plans as needed, and communicate with the interdisciplinary team
- Ensure patient and staff safety during interventions, adhere to infection control practices, and provide patient education and home exercise instruction
- Support discharge planning and coordinate with care team for smooth transitions between levels of care

Requirements
- Active physical therapy license in good standing for the applicable state
- Ability to work in a skilled nursing facility setting
- Demonstrated ability to document care plan progress and communicate clearly with staff
- Willingness to commit to a 17-week travel assignment with 35 guaranteed hours per week
