OurOutreach & Case Management Programaims to empower clients to become more self-sufficient by helping them prioritize their needs and create sustainable plans for long-term improvement. This program serves as a vital link between individuals in crisis and the services, resources, and community connections they need to thrive. What Will I Do as an Outreach Case Manager? In this role, you will: Develop individualized care plansbased on client needs assessments, identifying areas for financial, physical, and emotional support. Assist clientsin enrolling in services like Medicare, food assistance, disability programs, and more. Advocate for clientsby connecting them to appropriate resources during times of crisis and providing ongoing personal support. Monitor client progressand adjust care plans as needed to ensure positive, sustainable outcomes. Maintain accurate case records, logging significant events and tracking client progress. Collaborate withinternal programs and external community partnersto provide comprehensive care and ensure clients receive all necessary services. Qualifications: Bachelor’s degree in social work or a related field (strongly preferred) Two years of experiencein case management, family services, or social services is ideal Travel required– Valid driver’s license, reliable transportation, and vehicle insurance (minimum coverage: $100,000/person and $300,000/accident) Bilingual in English and Spanishis strongly preferred Salary Range: $45,000 - $47,000 Why Join Us?
